We accept CDM/EPC (723), DVA (D904), T2DM Group Session (81110, 81115) and Workers Compensation Referrals! You can refer to us by faxing the relevant forms to 4961 0677 or via post to Level 1, 58 Broadmeadow Rd, Broadmeadow 2292.

We know the difference is not in guaranteeing our results. We do, however so does every other health professional. We are all educated and excellent at our clinical skills. We know that what you want is COMMUNICATION. You want to know what’s going on with your patient, that they are being cared for and that they are engaged with their treatment plan.
So we have the following guarantees (feel free to call us out on these at any time)
We guarantee to have a written report to you within 48 hours of any patient referred by you (whether under a CDM/TCA or a private patient). Where possible, we will deliver this by hand.
We guarantee to return your phone calls, faxes and emails within 24 hours. We guarantee to inform you of any referred patient who has not attended, did not arrived or cancelled for no reason without rescheduling.
We guarantee to provide your patients with the opportunity to provide anonymous feedback at some point during the course of their treatment.
We will use this feedback to address concerns and improve our services.
