Transcend Health Professionals accept CDM/EPC (723), DVA (D904), T2DM Group Session (81110, 81115) and Workers Compensation Referrals! You can refer to us by faxing the relevant forms to 4961 0677 or via post to Level 1, 58 Broadmeadow Rd, Broadmeadow 2292.

Our Health Professionals don’t want to be like every other allied health provider out there. Transcend Health want to be Excellent, Exceptional. Transcend Health want to TRANSCEND your expectations.

Transcend Health know the difference is not in guaranteeing our results. Transcend Health do, however so does every other health professional. Transcend Health are all educated and excellent at our clinical skills. Transcend Health know that what you want is COMMUNICATION. You want to know what’s going on with your patient, that they are being cared for and that they are engaged with their treatment plan.

So Transcend Health have the following guarantees (feel free to call us out on these at any time)

Transcend Health guarantee to have a written report to you within 48 hours of any patient referred by you (whether under a CDM/TCA or a private patient).

Transcend Health guarantee to return your phone calls, faxes and emails within 24 hours. Transcend Health guarantee to inform you of any referred patient who has not attended, did not arrived or cancelled for no reason without rescheduling.

Transcend Health guarantee to provide your patients with the opportunity to provide anonymous feedback at some point during the course of their treatment.

Transcend Health will use this feedback to address concerns and improve our services.
