Informed Consent and Terms and Conditions

Please read:

• Service fees are due at the time of appointment.  
• For services to be paid by a third party (i.e. DVA, NDIS, CTP, SIRA), Transcend Health must have all relevant paperwork and information for billing purposes. Failure to provide Transcend Health with this information prior to the commencement of the appointment will result in me (the client) being charged the full-service fee.  
• If I am unable to attend the scheduled appointment time for any reason, I am required to contact Transcend Health on (02) 4961 3399 as soon as possible. Appointments are best rescheduled as soon as possible to avoid disruption to the treatment plan. 
• I acknowledge that the FULL APPOINTMENT FEE will be charged if I fail to give 12 hours prior notice to cancel a session. No-Shows will incur 100% of the service fee. This will be waived in exceptional circumstances. 
• Results require adherence to the treatment plan and results are dependent on the adherence of the client.  
• I acknowledge that it is my responsibility to arrive on time for my scheduled appointments, as sessions will finish at the original scheduled time regardless of late arrival.  

Questions of a personal nature: 

• I am aware that I will be asked for personal information relating to my health and how it impacts my life. Clear and accurate information assists my treating clinician to provide more effective treatment. It is my choice as to what information I choose to provide.  

You need to let us know: 

• The risk related to some exercise disciplines can increase if your clinician is unaware of certain facts, I am aware I need to inform my clinician if any of the following apply to me: 
• A pacemaker or cardiac condition, including recent cardia surgery 
• Suffered from blood clots, thrombosis or stroke 
• Suffer from diabetes 

• Are currently taking medications, and what they are 
• Have an injury, or history of injury, including recent surgery that may affect treatment 
• Are at risk of having a seizure, black out or other loss of control that could cause harm to myself or others 

Clinical Physical Contact: 

• I understand that during your sessions, assessment and treatment, it may be necessary for my treating clinician to make physical contact. I consent to Clinical Physical Contact and understand I can withdraw consent at any time, and which point physical contact will cease immediately.  

Risks related to training: 

• I understand with all treatments, there are risks and benefits. My treating clinician will discuss any foreseeable risks with me. I acknowledge these risks and will take care to heed the advice given 

Children and Minors: 

Consent from a custodial parent is required prior to the commencement of services. This applies to anyone under the age of 18.  

Practitioner Collaboration:  

By signing this form, you are agreeing that your practitioner may discuss your case with another practitioner, trainer or allied health professional for case conferencing or referral purposes.  

Photos, Videos, use of likeness, feedback for business promotion use: 

We may take photos, videos, written or verbal feedback of our sessions for use on social media and other marketing media. By signing this form, you are giving your consent to our use of these materials.
